To the LDS, if a person is "worthy", it means that person is "worthy to enter the temple". In a mechanical sense, it simple means that person has passed a quality check by answering a series of questions favourably in a temple recommend interview. This interview is conducted once every two years for every member. Each member is interviewed first by a member of the bishopric (or branch presidency) and then by a member of the stake presidency (or district/mission presidency). Both interviews are exactly the same.
These are the questions. I've provided the answers that would qualify a member as worthy:
1. Do you believe in God, the Eternal Father, in his Son, Jesus Christ, and in the Holy Ghost; and do you have a firm testimony of the restored gospel?
Correct answer: YES2. Do you sustain the President of The Church of Jesus Christ of Latter-day Saints as the prophet, seer, and revelator; and do you recognize him as the only person on the earth authorized to exercise all priesthood keys?
Correct answer: YES3. Do you sustain the other General Authorities and the local authorities of the Church?
Correct answer: YES4. Do you live the law of chastity?
Correct answer: YES5. Is there anything in your conduct relating to members of your family that is not in harmony with the teachings of the Church?
Correct answer: NO6. Do you affiliate with any group or individual whose teachings or practices are contrary to or oppose those accepted by The Church of Jesus Christ of Latter-day Saints, or do you sympathize with the precepts of any such group or individual?
Correct answer: NO However, this is a really vague question... What does it mean to 'sympathize' with a group or individual? And what groups are we talking about?
7. Do you earnestly strive to do your duty in the Church; to attend your sacrament, priesthood, and other meetings; and to obey the rules, laws, and commandments of the gospel?
Correct answer: YES8. Are you honest in your dealings with your fellowmen?
Correct answer: YES9. Are you a full-tithe payer?
Correct answer: YES10. Do you keep the Word of Wisdom?
Correct answer: YES This only covers coffee, tea, tobacco and drugs, and alcohol. Caffeinated beverages are fine.
11. Have you ever been divorced or are you now separated from your spouse under order of a civil court? If yes,
(a) - Are you current in your support payments and other financial obligations for family members, as specified by court order or in other written, binding commitments?
Correct answer: YES (b) Were there any circumstances of transgression in connection with your divorce or separation that have not been previously resolved with your bishop?
Correct answer: NO12. If you have received your temple endowment --
(a) Do you keep all the covenants that you made in the temple?
Correct answer: YES (b) Do you wear the authorized garments both day and night?
Correct answer: YES13. Has there been any sin or misdeed in your life that should have been resolved with priesthood authorities but has not?
Correct answer: NO14. Do you consider yourself worthy in every way to enter the temple and participate in temple ordinances?
Correct answer: YESAn 'incorrect' answer will result in further questioning, and may result in you not receiving a temple recommend, at which point you are 'unworthy to enter the temple'.
You asked if you could help members become worthy again. I'd suggest that worthiness is a very personal decision. Many things can lead to unworthiness, some of which is desired by the member. For instance, I no longer believe that the prophet and apostles are indeed prophets and apostles as the church defines them to be. Does that make me unworthy to enter the temple? Absolutely. Do I require counseling and reform and repentance? Not at all.
