The last recorded "official" revelation/prophecy is dated 1918 and can be found in the Doctrine and Covenants.
Section 138
A vision given to President Joseph F. Smith in Salt Lake City, Utah, on October 3, 1918. In his opening address at the 89th Semiannual General Conference of the Church, on October 4, 1918, President Smith declared that he had received several divine communications during the previous months. One of these, concerning the Savior’s visit to the spirits of the dead while His body was in the tomb, President Smith had received the previous day. It was written immediately following the close of the conference. On October 31, 1918, it was submitted to the counselors in the First Presidency, the Council of the Twelve, and the Patriarch, and it was unanimously accepted by them.

Since that date there have only been three official/formal statements that can be considered as relating to revelation/prophecy.
1. The 1890 manifesto announcing the end of polygamy.
2. The 1978 statement announcing the end of racial discrimination in terms of Priesthood holding and Temple attendance.
3. The 1995 statement stating the Church's position on The Family.
1& 2 allude to revelations being received but, unlike previous inserts into the D&C, the actual revelations themselves have not been recorded.
3 is not claimed to be the result of a revelation.
So, to all intents and purposes, Revelations and Prophesies that were of such significance that they be canonised ceased in 1918.
Have they actually stopped, or has the Church just stopped publishing and canonising the words of the actual revelations and prophecies?
Why has official revelation stopped...?

Why has official revelation stopped...?
That said, with the Book of Mormon, we are not dealing with a civilization with no written record. What we are dealing with is a written record with no civilization. (Runtu, Feb 2015)

Re: Why has official revelation stopped...?
Wait? Are you saying the much hyped and anticipated historic missionary announcement where there revealed that stateside missionaries would stop door knocking and start driving chairs in ward houses to proselyte on Facebook was not revelation?

Revelation can be subtle or are you forgetting the demise of the Glenn Beck show or changes at the Maxwell.
